Exemestane – Aromasin

This is Exemestane, sold as Aromasin, made by Syncom Labs at 20mg per tablet. It is a steroidal aromatase inhibitor that binds to the aromatase enzyme irreversibly, permanently disabling it rather than just blocking it temporarily. This is why it does not carry the same oestrogen rebound seen with reversible AIs when they are stopped. It is used alongside testosterone-based protocols to keep oestradiol from climbing too high.

As a guide and not medical advice, a common starting point is a quarter of a 20mg tablet, around 5mg, every other day, taken with food to help absorption, adjusted according to bloodwork and how the person feels rather than a fixed schedule. Doses are typically kept well under 12.5mg a day. Any real dosing decision should be based on your own oestradiol and lipid results, not a generic number online.

Not everyone does. Aromasin is used to manage oestrogen-related side effects that can come with aromatising compounds like testosterone, such as water retention or gyno symptoms, so it is a support compound rather than a base one. Whether it is needed depends on individual sensitivity and bloodwork, which is why baseline oestradiol testing before adding an AI is worth doing.

Reported effects include joint pain, lowered HDL cholesterol, fatigue and headaches. Lipid panels are worth checking alongside oestradiol, since pushing oestrogen down too far can affect cholesterol and joint comfort. Liver enzymes can be checked as a general precaution.

Aromasin is typically paired with a testosterone base, since it is used to control the oestrogen that testosterone aromatises into rather than as a standalone compound. Many people also add liver support alongside it, given the extra hepatic load that can come with oral compounds.
